Statement of Death Form SSA-721
Overview
The Statement of Death form is a Social Security Administration form (form SSA-721) used to verify a death so survivors of the deceased family member can apply for social security benefits.
There are two versions of this form to select from in MIS Professional:
- SSA721 - with Informant Name & Address
- SSA721
- This version of the form leaves the box below the "Name of Deceased" field empty to be manually populated
To run this report:
1. Navigate to Reports - Reports Selection
2. Select the "SSA721" form from the list
3. Click Run
